The head of RSA Ireland leaves just over two weeks after investigation is launched.

Philip Smith, chief executive of RSA Ireland, has resigned and issued a statement claiming to be the "fall guy" for the issues at the provider.

According to reports in the Irish media, Smith's statement also claims that the insurer's investigation has a "predetermined outcome". The reports added that Smith said he and his family had been traumatised by the events.

Smith was suspended by RSA earlier this month as it launched an independent probe into claims and finance issues.
