I fear for the reputation of insurance. One major insurer, it is
reported, has been erroneously informing private medical insurance
customers that their policies have been cancelled and accusing others of
not paying premiums - all because of faults in its systems.


Policyholders complain that there is no apology forthcoming even after the
error is corrected. Other policyholders complain of alterations to policy
cover without previous notice. Why do things like this happen?
